If you're interested in enrolling your three or four-year-old child into Westfall's Preschool program, please complete the 2023-2024 WES Preschool Interest Form before February 9, 2023.
This interest form is the first step to applying for preschool! However, completing this interest form DOES NOT GUARANTEE your child's enrollment in our program.
Completing this interest form will allow the Westfall Preschool Coordinator to prioritize the ages and needs of the children.
The Preschool Coordinator will begin to set up screening appointments and collect income verification in March 2023. Once the children's screenings are complete and income is verified, eligible families will receive a tuition notice and a full enrollment packet to be completed and submitted by April 15, 2023.
Once provided with the full enrollment packet, you will return the completed packet with the following documents to be copied for your child's enrollment file:
- Child's Birth Certificate
- Child's Social Security Card
- Child's Immunization Record
- (2) Proofs of Residency (rental/lease agreement or mortgage coupon, a utility bill ( only one utility bill may be used), or an Ohio Driver's License with current address.)
- Completed Physical Form
- Completed Dental Form
All completed preschool enrollment packets must be returned to the front office by April 15, 2023.
